Refer a Patient

Thank you for referring your patients to Flagstaff Medical Center. Our goal is to make the referral process as easy as possible for physicians. FMC uses a Patient Placement Center system to streamline admissions and communication between physicians.

FMC offers several Outpatient Services. To refer your patient for Imaging, Cardiopulmonary or Laboratory services, please completely fill out an Ancillary Services Order Form.
