FMC supports North Country HealthCare Learning Center | The FMC Foundation supports targeted initiatives at FMC and in the greater Flagstaff community by providing funding through its Foundation Awards Program. Funding requests (internal and external) should address community healthcare needs, adhere to the core mission of the hospital or reflect projects that are deemed to be in the best interest of the hospital.
To be considered for an award, please review the Funding Guidelines and submit a proposal addressing the identified questions. Your proposal may be faxed to 928 773-2549 or mailed to:

The Foundation Committee meets six times per year. Applicants are notified of the outcome by mail.
For those who receive an award, a final report must be submitted to the FMC Foundation within one month of the program’s conclusion. The report should include:
- Program summary with number of individuals served
- Evaluation of program’s effectiveness and feedback from participants
- Other pertinent information regarding the program’s success
- A high-resolution, digital photo with permission to use as needed in promotional efforts
