Funding Request Guidelines
Funding requests (both internal and external) should address community healthcare needs, adhere to the core mission of FMC, and/or reflect projects that are deemed to be in the best interest of the hospital. Applicants must be a non-profit organization under 501(c)3 of the Internal Revenue Code.*

Proposals should include answers to the following questions:
- What is the specific program for which the funds are being requested?
Needs for Northern Arizona and requests for one-time funding will be given priority.
- Who will the program serve and how many individuals do you expect to benefit?
- How will the program provide measurable improvement?
- Are other agencies supporting this project? Include explanation.
- How is the program consistent with FMC’s mission?
- What marketing efforts will be utilized in support of the program?
- What is the overall budget for this project?
- If awarded, how will the funds be used and how will they make a difference to those served?
- Identify any previous funding this program has received from the FMC Foundation.
Note: Proposals are reviewed monthly.
At the conclusion of the program, a final report must be submitted to the FMC Foundation including:
- Program summary with number of individuals served
- Evaluation of program’s effectiveness and feedback from participants
- A high-resolution, digital photo with permission to use in promotional efforts as needed (digital photo may be e-mailed to Natalie.Harlan@nahealth.com)
- Other pertinent information regarding the program’s success
